RN - Home Health/Hospice - PT

32 hrs/wk; 8a-4:30p, variable days/wk. Direct clinical nursing services in clients home/residence or facility, including but not limited to IV therapy, venipuncture, wound cares and wound vac, Hospice, coordination of care between MD, facility, patient/family and rehab. Will take week day and weekend call on rotating schedule. Must be able to work independently. Excellent communication and customer service skills, strong computer skills, team oriented with previous nursing experience needed. BSN or willing to obtain, NE RN license and BLS. Preferred candidates will have home health/hospice experience. Able to drive own vehicle, valid drivers license and car insurance.
